Having trouble paying your phone bill? If you are a resident of Minnesota, who lives in a low-income household or participates in one of the programs below, you may qualify for a $10.00 monthly credit on qualifying landline telephone services through the Minnesota Telephone Assistance plan.

Medical Assistance (MA) / Medicaid
Supplemental Nutrition Assistance Program (SNAP)
Supplemental Security Income (SSI)
Veterans Pension or Survivors Pension
Federal Public Housing Assistance
Bureau of Indian Affairs General Assistance
Tribal Temporary Assistance for Needy Families
Head Start (income-qualified participants)
Food Distribution Program on Indian Reservations (FDPIR)

Income-Based Eligibility*

Household income is at or below 135% of the Federal Poverty Guidelines.

You must provide proof via one of the following:

Last year’s state, federal, or tribal tax return
Social Security benefits statement
Current employer annual income statement
Retirement/Pension statement
Three most recent pay stubs
Divorce decree
Child support document
Unemployment or Worker’s Compensation statement
Veterans Administration benefits state

Application

Complete and mail the application form to 101 3rd Avenue SW Cedar Rapids, IA 52404 with necessary files. If approved, it can take up to two months for the discount to show up on your bill. You must pay the whole bill until then. Discount will not cover past due bills.

You must renew on a yearly basis to maintain the credit with a resubmission of the form & needed documents through mail.
